PharmAdva, LLC strives to protect the confidentiality and privacy of its users to the greatest extent possible. In order to do so, all of the protected health information (PHI) collected about the users that are communicated between the medication dispensing unit (MedaCube), the on-line systems, the staff, and the caregivers that the user authorizes, is stored and transmitted in a confidential manner meeting the Health Insurance Portability and Accountability Act (HIPAA) standards. To maintain the users’ confidentiality as best as possible, the only identifying information provided during information transmissions is the identifying number associated with the medication dispensing unit. In addition, user records maintained by PharmAdva, LLC will be kept in a locked file or otherwise secured storage location, such as a secured server, at all times, except when being reviewed or updated.
PharmAdva, LLC employees are bound by ethical and legal codes to protect the confidentiality and privacy of our users and to protect and maintain the confidentiality of all information learned about users, their family members and acquaintances in the course of providing services to them. Confidential communications include conversations, reports, forms, correspondence, and computer generated communications with, about or involving in any way any user of PharmAdva, LLC. Access to information is limited to an “as needed/need to know” basis. Employees authorized to view user (patient) information have agreed to keep all such information absolutely confidential and understand that it is not be discussed at any time with any person under any circumstances, unless an employee needs to do so in order to carry out his or her job duties, or unless the person discussing the information is talking or otherwise communicating with the subject of the information (or their authorized caregiver) at that person's (or their authorized caregiver’s) invitation. Any employee who is found to have discussed medical information about a user with anyone else in violation of this policy, or who is found to have released such information without authorization, will be subject to severe disciplinary action, up to and possibly including immediate termination from employment. In addition, state and federal laws may subject such an employee to both civil and criminal action in a court of law.
PharmAdva, LLC collects medication usage information and, if instituted on your MedaCube, other health information input into the MedaCube such as responses to questions provided through the MedaCube interface including, but not limited to, pain frequency and severity, anxiety and depression scales, etc. and measured parameters including, but not limited to, weight, blood pressure, heart rate, oxygen saturation, glucose level, etc. to provide services to our users. We use the information we collect to provide, maintain, protect and improve our services to our users. We do not share personal information with companies, organizations and individuals outside of PharmAdva and our Business Subcontractors and Healthcare Partners unless we have the patient’s explicit consent or as required by law. We may share non-personally identifiable information publicly or with business customers.
Our Privacy and Confidentiality Policy may change from time to time. We will not reduce your rights under this Privacy and Confidentiality Policy without your explicit consent.
